I certify that I: (a) have completed the applicable requirements for a Pilot's Certificate issued by the Federal Aviation Administration or an equivalent foreign agency, (b) hold or have held an FAA Pilot Certificate or foreign equivalent for a fixed-wing, rotary-wing, sport, balloon or glider aircraft, and (c) have exercised the privilege as a pilot in command of such an aircraft after reaching the age of 80. I hereby agree to abide by the Charter and Bylaws of the United Flying Octogenarians.
